The Firefighter Mayday Survey is a variation and continuation of Project Mayday, originally started by the late Don Abbott.
In order for the fire service to continuously improve, better understand firefighter experiences, and equip individuals with meaningful intelligence, it is imperative that we have an opportunity to objectively review incidents that have occurred.
The more insight we have to analyze and review details about these occurrences, the more potential we have to positively impact our industry and reduce preventable injuries and/or deaths.
Our efforts officially launched on January 1, 2025, after several months of piloting. We are also collaborating with the Firefighter Rescue Survey team to mirror a similar approach to data collection and correspondence.
We will objectively collect, compile, and analyze the information, sharing both the results and raw data with the entire fire service for free. Our goal is to enhance firefighter safety, training, and fireground tactics.
You should complete a survey anytime a firefighter is in distress (or believes they may be) during a structural fire, regardless of whether a Mayday was declared. This includes situations involving a firefighter in distress, a near-miss, a close call, or a Mayday event.
Absolutely! Regardless of the severity, complexity or outcome you should fill out the survey.
We prefer that someone who operated at the incident scene completes the survey. However, anyone can fill it out. All information will be validated and cross-examined regardless.
Yes, it can. You only need to provide the date, and location in order for us to validate the entry.
No, we do not disclose any private or personal information. All data is collected and shared in a way that ensures the anonymity and privacy of individuals involved. Our focus is solely on gathering information that will improve firefighter safety, training, and tactics without compromising personal privacy.
Yes, absolutely. We aim to gather as much data as possible for thorough analysis and cross-referencing. Once we have a robust set of raw data, you’ll be able to filter and examine it by year.
No. You can submit a survey as soon as you feel comfortable.
If none of the options fully capture your response, choose the answer that best aligns with your perspective. Select the closest option available, even if it’s not an exact match. The survey will be followed by a phone interview where you can explain exactly what happened.
